Many Asian patients are interested in the "double eyelid operation".
This is a procedure which creates a more definite upper lid crease or fold, giving a slightly occidental appearance. The upper eyelid in Asian patients has specific anatomical differences from the occidental upper eyelid, with orbital adipose tissue coming further forward in the lid resulting in an absent or low crease. In addition, there often is a prominent epicanthal fold (a fold of skin in the inner aspect of the eyelids near the nose), which must be taken into account. There are variations in the appearance from no fold, to a faint fold, to a doubled or tripled fold, to this plus prominent bulging above the fold (which can also be repaired at the time of surgery).
The key to Asian blepharoplasty is in creating a natural appearing fold at an appropriate height in the lid.
If this is too high, the appearance will be too occidental. Of course, post-operative symmetry is important (having the two eyelids in appearance). In order to achieve an effective crease, it often has to be made "over-emphasized" for the first few weeks.
It is important to have an experienced surgeon who knows how to deliver exactly what is desired.
I often have patients bring in a movie magazine close up of their favourite movie star to show me how high they would like the crease (since most of these actors have had the procedure done). Patients need to understand that even in experienced hands, there is some variability to the results.
Besides creating a more definite crease in the upper lids, older Asian patients may wish to have excess skin and bulging or puffiness in the upper
lids reduced, and puffiness in the lower lids also eliminated at the same time.
Because of his experience, training and skill, Dr. Oestreicher can occasionally improve on unsatisfactory results from surgery done elsewhere, and this includes Asian blepharoplasty. For instance, indefinite folds, uneven folds or unequal folds can sometimes be improved.
Every case is different and must be examined individually. It is possible to emphasize or raise the height of an eyelid fold but it is extremely difficult to reliably lower eyelid folds. Dr. Oestreicher would be pleased to examine your eyelids and listen to your concerns.
